The launch of frameworki into Community Health is a logical step that builds upon Corelogic’s success in Wandsworth Primary Care Trust as well as on national projects including SCPEA (Social Care Personal Demographics Service Early Adopters) and CAFA (Common Assessment Framework for Adults). Our pioneering work in key healthcare areas such as community nursing, older people’s services, intermediate care and specialist services (such as cardio and neuro rehabilitation) has proven the value of our solution to Community Health. Moreover the integrated nature of the solution means that a shared record is accessed by all professionals across the social care and health divide, leading to better informed decision making and ultimately better patient care.
frameworki is a highly usable, strategic, integrated and configurable solution that includes support for Healthcare, Adult Social Care and Personalisation, Children’s Services and ICS, financial management and control, reporting, and mobile working.
In Community Health, frameworki enables a holistic view of a patient’s case and associated health recording to deliver timely information sharing between district nurses, intermediate care staff, therapists and social care practitioners in support of more efficient and effective decision-making. It also captures activity information on an individual and clinic basis for ‘payment by results’ data needed for financial planning and funding allocation.
frameworki is already used by 600 health staff in Wandsworth PCT, with over 80,000 activity recording episodes completed to date by nursing and intermediate care staff alone.
